How to create a custom form
How to create a new custom form
To create a new custom form, open the Custom Forms module and select Create New in the top-right corner. The Add New Custom Form modal window will open. Complete the fields and click Save. You will be redirected to the Form Detail page on the Summary tab to begin designing the form.
- Internal Name (required) – The internal identifier for the form.
- Title (required) – Auto-populated from the Internal Name; this is the end-user display title (you can edit it).
- Description – Optional details or instructions for submitters.
- Companies – Select the companies this custom form relates to.
After saving, you will be navigated to the Form Detail page. Proceed to design the form layout, sections, and input fields.
Form navigation header tabs
- Summary – Displays form details and an editable form preview.
- Design – Lets you add and arrange sections and fields, configure actions, lookups, and file attachments.
- Preview – Shows exactly how the form will appear on the web.
- Entries – Lists submissions (Name, Company, Submission Date, Entry Reference Number, etc.); use the action icon to open entry details. Customize visible columns using the gear button in the top-right corner.
- Shared With – Shows Shared Spaces and Participants; use the row action menu to revoke access or the Share button to add sharing. The Public Links section lists Title, Expiration Date, State, Password Protection, Created by/when, and Last Activity; use the … menu to edit, deactivate, delete, copy link, or copy password. Use Add New to create a public link for external sharing.
- Users – Displays users and their roles; use the row action menu to add roles and the Grant Access button to give access.
- Settings – Lets you configure Entity, Board, Entry Reference format, On Entry Status, and On Update Status for the form.
